Membership Information

• Benefits of Membership

GAMA membership is open to those licensed physicians (MD or DO), Residents and Interns living or working in the greater Albuquerque area. 

Dues for Active membership in GAMA and the New Mexico Medical Society (NMMS) for 2009 are $205 for county and $325 for state.  Membership for Interns, Residents, and Fellows is $60/yr (GAMA is $35/yr and NMMS is $25/yr). Although membership is voluntary, membership in both GAMA and NMMS is a condition required by the Bylaws of the State Society.  Dues are billed after an applicant has been approved for membership by the Board of Trustees. 

We have tried to make the application process as easy as possible. You may either contact GAMA (505-823-4760) for an application, or take advantage of the online application .

Online Applications

• GAMA Membership Application (MD or DO) is a Adobe Acrobat file (pdf) your application will be processed and sent to the GAMA Board of Turstees for appoval when the application is complete. (signature, picture, cv....). 

Note: The form may be signed with an electronic signature. Follow the instructions to electronically sign the form when you click in the signature line. File may also be saved on your desktop so that you may fill out the application and attach the file on to your e-mail application. If you have any questions you may contact us at 821-4583. Note: You may also fax your application to 823-4760 or mail your application to GAMA, 7770 Jefferson NE, Ste 420, Albuquerque, NM 87109. If you fax or your application please fax us a seperate CV from the form. A photo may be e-mailed to us in jpeg format at 300dpi. gama@gamamed.org
